OUR OFFICE POLICIES
The office hours for individual physicians and ancillary departments do vary according to the needs of their patients. Each physician has a direct telephone number. For better service and office hours, you are encouraged to call your physician’s office directly.
Calls regarding appointments and prescription refills should be handled during regular office hours. If you need to contact your physician after hours, an answering service, Unique Personal Communications, 419-887-5300, will answer calls for Toledo Clinic physicians and contact your physician if necessary. If this is a medical emergency, please call 911.
FOR APPOINTMENTS
In order to provide the best care for our patients, appointments are required for all office visits. To schedule a visit with your physician, call that office directly. As a courtesy to other patients, please give 24-hour notice to your physician if you are unable to keep your scheduled appointment. Upon request each medical office has a fee schedule available for patients.
UPON YOUR FIRST VISIT
If this is your first visit to The Toledo Clinic, you should arrive at least 15 minutes prior to your scheduled appointment. This will allow the Clinic time to complete the necessary registration information. When you arrive at The Toledo Clinic, enter the building in which your physician is located and check-in at the Information Center. It is important that you bring your health insurance information, including your insurance card and a photo ID.
FOR FUTURE VISITS
For subsequent office visits, you may go directly to the medical floor where your physician is located. Check in at the Information Center on that floor and they will notify your physician that you have arrived for your appointment. Important: Be sure to notify the Information Center staff of any change in name, address, dependents and insurance coverage. At each Information Center a suggestion box is available for patients to give ideas for improving patient services.
MEDICAL RECORDS
Whether you have visited one or several physicians at The Toledo Clinic, you will have one chart. Medical records are kept in one central office, which allows you and your Toledo Clinic physicians immediate access to your complete medical history. Medical records are strictly confidential. Your records cannot be released to someone other than your physician without your written permission.
ABOUT YOUR MEDICAL BILLS
Your monthly statement encompasses professional services provided by The Toledo Clinic Physicians. You will be billed separately by the hospital for room and other hospital-related charges. As a convenience to our patients, a patient representative will be available to answer your questions regarding your account balance at 419-479-5398.
TOLEDO CLINIC INSURANCE POLICY
You do not need to file a claim to your primary or secondary insurance carrier if the message “Insurance claim filed with (your insurance company name)” appears on your monthly itemized statement and we have received direct payment from your primary carrier.
TOLEDO CLINIC CREDIT POLICY
All outpatient visits should be paid on the day of the visit. The Toledo Clinic policy requires a minimum payment of $25.00 each month on a balance of less than $100.00. A balance larger than $100.00 requires a monthly payment of at least one fourth (25%) of the original charges.
Your insurance policy is a contract between you and your insurance company. It is important that you understand its provisions. Our physicians contract with various provider organizations and we will honor those agreements. In those situations appropriate contractual adjustments will be made following receipt of payment.
